Registration using Google Forms
This new registration form is intended for anyone interested in participating in ANY of our Second Chances Community Music programs. It is a hyperlink that will lead you to a Google Form asking for the information we need to complete your registration for the program. We will be using the same form for ALL of the following ensembles:

- Second Chances Intermediate Band
- Second Chances Symphonic Band
- Second Chances Jazz Band (8wk program)
- Second Chances Jazz Combo Program (8wk program)
- Second Chances Singers: Intro to Sight Singing (8wk program)
All information submitted using this Google form will be kept confidential and will be used by the Second Chances Executive Director, the Second Chances executive (President, Vice President, Secretary, Treasurer) and by our program’s Registration Coordinator only.
