Registration using Google Forms
This new registration form is intended for anyone interested in participating in our Second Chances Community Music programs. It is a hyperlink that will lead you to a Google Form asking for the information we need to complete your registration for the program. We will be using the same form for ALL of the following summer programs:

- Second Chances Beginner Band – NO SUMMER PROGRAM OFFERED
- Second Chances String Orchestra – NO SUMMER PROGRAM OFFERED
- Second Chances Small Ensembles / Chamber Music Program – NO SUMMER PROGRAM OFFERED
- Second Chances Intermediate Band – runs Tues 7-9pm for entire summer
- Second Chances Symphonic Band – runs Wed 7-9pm for entire summer
- Second Chances Jazz Band w/Jeffery Mosher – runs Thursdays 7-9pm for 11 weeks
- Second Chances Jazz Combo w/Jeffery Mosher – runs Thursdays 6-7pm for 11 weeks
- Second Chances Jazz Workshop w/Peter Bull – June 2, 9, 16 and 23rd only
- Second Chances Singers – program will run until June 30th
All information submitted using this Google form will be kept confidential and will be used by the Second Chances Executive Director, the Second Chances executive (President, Vice President, Secretary, Treasurer) and by our program’s Registration Coordinator only.
